How to Request Your Accommodation Letter

Returning students who are registered with Accessible Education Services must request an Accommodation Letter every semester.

When using the webform:

  • Make sure to ADD the Professors you want me to send your Accommodation Letter to.
  • Use the ADD MORE button in the bottom right corner to make sure all of your Professors for the entire semester are added.
